Case Manager Assistant - Full Time

Description

The Case Manager Assistant (CM-A) is responsible for assisting the Case Managers with a verity of health care service deliveries and discharge planning with the multidisciplinary teams input. The Case Manager Assistant will play a vital role in obtaining necessary resources and distributing documents to responsible stakeholders.

  • Collaborate with Admissions Team and Clinical Liaisons to prepare pre-admissions paperwork
  • Access lab reports, medications, and therapy notes to accompany reports to stakeholders.
  • Fax reports to stakeholders
  • Fax orders to (Durable Medical Equipment) DME and suppliers
  • Review medical bills for life-care patient for accuracy, forward to appropriate stakeholders or guardian for payment
  • Complete discharge satisfaction surveys
  • Assist with discharge planning for resources and placements, check on placements accepting of payors, bed availability, services provided, cost, etc.
  • Fax clinical information to stakeholders
  • Build initial Case Management file on scheduled admissions; face sheet, insurance info, medical records, blank release of information form, and patient labels.
  • Thin and expunge old Case Management files
  • Maintain campus wide recertification list
  • Provide back-up for treatment planning schedule/changes & distribution
  • Back up for note taking in treatment planning conferences
  • Assist Case Managers with preparations for treatment planning conferences; printing last MTP, therapy notes, sign-in sheets.
  • Complete a six month follow up call to all patients/guardians for a status update. 
  • Performs other duties as assigned.

Job Requirements

Experience

  • Two (2) years of case management experience or healthcare experience preferred.

Education Required

  • High school diploma or equivalent required.
