Whether a person is challenged by a traumatic injury, chronic illness, or rare condition, they may require long-term rehabilitative care and ongoing medical services. Those with a lengthy hospitalization or history of hospitalization can benefit from a stable, home-like specialty hospital to continue making meaningful progress.

Offering a haven in a time of need, Nexus’ inpatient care setting is secure and comfortable. Patients and their families are in good hands as they weather the ups and downs of receiving care for complex diagnoses.

From pulmonary services and ventilator weaning to treatment of severe infections and wound care, to counseling and rehabilitative therapies, Nexus is capable of addressing a variety of complex needs. The Harbor Program helps individuals gain the confidence to brave what lies ahead and find their course of healing with us, because we’re focused on mending minds.

An uncommon combination vital to whole person care — a complex care program

Nexus offers holistic care, including primary care physicians, neurology, physical medicine, psychiatry, rehabilitative therapists, dietitians, case managers, social workers, and more. While other hospitals may have a step-down ICU, Nexus offers a range of services under one roof to adapt our approach as patients progress through their recovery journey.

A therapeutic setting helping complex, technology-dependent individuals

More than a rehabilitation facility, Nexus’ complex care program manages central lines, feeding tubes, g-tubes, and other lines, and cares for ventilator-dependent individuals and those with ventilator-weaning orders. Central monitoring to continuously evaluate our patients gives families peace of mind, while our patient advocates work to ensure all needs are met. In conjunction with medical and rehabilitation services, Nexus also offers behavioral and mental health support — which ensures the whole person is cared for and coping well.

If your loved one or patient has experienced a serious illness or a traumatic injury, finding a specialty hospital that offers both medical and behavioral care is crucial for successful recovery. Nexus is equipped to provide comprehensive inpatient care as patient needs evolve. From admission to discharge, our team guides patients and their families through a personalized rehabilitation journey, including physical therapies, psychological evaluation and treatment, medication management for behavioral challenges, caregiver training and education, and a step-by-step plan for discharge.

Supporting families of loved ones who have a serious illness or injury

Caring for a loved one who has experienced a traumatic injury or serious illness can be overwhelming. Caregivers often struggle with worry, unwanted thoughts, and feelings of helplessness and anxiety. At Nexus, we provide caregivers the training and support they need to be successful when it is time to go home. Our counseling services also help patients and their families manage their feelings and reactions.

Here are some essential tips we share with families:

  • Don’t neglect self-care. You can’t provide adequate care for your loved one without making sure your own physical and emotional needs are met.
  • Set realistic expectations for your loved one’s rehabilitation progress.
  • Communicate openly with your loved one. Encourage them to share and validate their emotions when they do.
  • Maintain strong connections with the rest of the family, including siblings and significant others.
