Diagnostic Evaluation Program

Nexus Specialty Hospital collaborates with physician offices, home health agencies, skilled nursing facilities, assisted living facilities, and other community agencies to provide patients with short-term support in a more intensive care setting.

Partnering with these organizations, Nexus meets patients’ needs in our ICU, high observation, or med/surg care settings and through outpatient services. Potentially eliminating the need for multiple and extended hospital stays or emergency room admits, Nexus helps patients return back to their original care environment.

The original care team is encouraged to visit the patient during his or her stay and participate in weekly team conference with Nexus case management staff. Clinical information is appropriately shared to optimize patient care.
