360 Care Coordination

Through 360 Care Coordination, we identify patients with particularly complex needs and provide training and assistance to caregivers. Our goal is to support the transition from hospital to home, ensuring successful outcomes long after discharge.

At Nexus, we believe it takes a village to heal the whole patient. That’s why we support families and caregivers in Texas throughout every phase of the admission and treatment process. Everyone from our patient advocates and case managers to registered nurses and physicians are available to answer questions and offer guidance as your loved one progresses through their program. We also offer an average of 60+ hours of caregiver training, so you’re fully prepared for life after discharge.
