Across the Nexus network, our wound care team is comprised of clinicians with specialty training and certifications in wound management — which means patients and residents are in great hands as we’re caring for their bodies and mending minds.
What is wound care?
Wound care focuses on the treatment and management of wounds to promote healing, prevent infection, and minimize complications. It encompasses the assessment, cleaning, and dressing of various types of wounds, including acute injuries such as cuts or burns, as well as chronic wounds such as pressure ulcers, diabetic ulcers, and venous leg ulcers. Wound care aims to facilitate the body’s natural healing process while addressing any underlying health issues that might impede recovery, such as diabetes or poor circulation.
One essential aspect of wound care is wound assessment, which involves the evaluation of the size, depth, appearance, and severity of wounds. This assessment, combined with the patient’s medical history and overall health, guides our treatment planning. Treatment can range from simple cleaning and dressing changes to more complex interventions such as debridement (the removal of dead tissue) or advanced therapies, such as negative pressure wound therapy.
Cleaning and dressing wounds are the cornerstones of effective wound care. Proper cleaning helps to prevent infection and prepare the wound for optimal healing. Dressing the wound serves several purposes: it keeps the wound moist, which is crucial for healing, protects it from external contaminants, and absorbs excess wound fluid.
At Nexus, our dedicated wound care team focuses on skin integrity, prioritizing the cleanliness of acute or chronic wounds that have previously resisted healing. Within 24 hours of admission, individuals are assessed by a wound care clinician to ensure all wounds are identified in a timely manner and an individualized plan of care is initiated. Although many protocols are in place to safeguard against new wounds, if they do develop, the team is trained in early detection and a treatment plan will begin right away.
